part of me thinks this is a hoax. someone in that state of mind doesn’t seem likely to appear on a talkshow.

That doesn’t confirm anything. That’s already been known that Casey Affleck is doing a documentary. The biggest evidence for a hoax right now is that if you watch the whole Letterman interview, at the end, he gets up and takes off his glasses and leans in to talk to Dave. That’s a bit out of character judging by his demeanor during the interview.

At 0:19 (“In what way?”), that seemed to be legit crazies. He perceives ambiguities in everything, so he has a difficult time speaking.
